Paying for Treatment: All individuals must receive a financial assessment prior to being admitted to an APT clinical program. The APT Foundation accepts payment from almost all entitlements or insurers who cover our services. This includes Title XIX, DMHAS General Assistance, and all Medicaid managed care plans which participate with the Department of Social Services program for TANF recipients.  For those with no third party coverage, APT uses a sliding scale to determine fees based on each individual's income and number of dependents. Individuals who do not meet the eligibility requirements of self-pay entitlements, or commercial insurance, may be eligible to enter a research program.

  • Community Health Network
  • Physicians Health Services (Medicaid)
  • Value Options
  • Compucare
  • Title XIX (Non-Managed Medicaid)
  • State Administered General Assistance (SAGA) through ABH

Financial Assessment :

The following list includes items required for a financial assessment to be done at intake:

  • Three current consecutive pay stubs
  • The previous year's W-2 Income Tax form
  • Medicaid/Medicare cards/forms
  • Proof of Insurance
